prepare files for 1.4.0

Fixes include dropping index from URLs. Bug fixes. HTTP protocol detection. Phing build system updates.

The issue was already on the 1.3.1 (next Milestone) so I pulled it in.

Also I renamed the milestones 1.3.1 → 1.4.0 and 1.4.0 → 1.5.0

I created a new 1.4 branch from right before #182. There's only one open code issue #146/#181. If this is reviewed and closed we should release the result as 1.4.

Last open 1.4 PR is merged now. #158 will be resolved after the release. If nobody vetos I'll throw it out over the weekend.

In regard of the change-log file: imho keeping a manual log-file about every git commit is an unnecessary burden. Let's just link to the appropriate comparison on github.
