Add a report to show rejected entries and the reason for rejection

What steps will reproduce the problem?
1. Duplicate entries in the input file
2. Adding a file that contain entries that are already in the database
3.

What is the expected output? What do you see instead?
Would like to be able to view a report of rejected entries that are either 
sorted by or have individual rejection cause information.

What version of the product are you using? On what operating system?

For text file input -- fixed and verified in r59.  

The reason for rejection is available in the wizard summary screen.

The reason for rejection is also available in the log file.
