Result titles sometimes blank #194

In some cases the title of a result is blank - this may be because we're
trying to use an empty Title field from a Word doc. We should have a
fallback system to make sure something is always shown, even falling back
to the document filename.

This seems to be due to the IFilter code in w32com_ifilter.py not reading Value
chunks correctly, or mapping them into the right metadata field names. Some
improvements made, but still needs tidying up. Visible with PDF files only 
AFAIK -
with Office files the OLE Storage interface is used which works fine.

The title now shows the the document title and the file path of the original 
file.
