Allow attachments to a page through the use of "__ATTACHMENT__" token

Allow attachments to a page through the use of "__ATTACHMENT__"
token when that specific page was NOT configured to have attachments
through either namespace or, category name configuration settings.

Magic word __ALLOW_ATTACHMENT__ was added instead to __ATTACHMENT__ as part of 
issue #53 implementation.  Rest of the implementation is complete and tested 
it.  Seems to be working without any issues -- however is not as clean as I 
like it to be.

This is overriding excluded pages settings which it should not.
