Refactor out HistoryNode inner classes #348

Refactor out TracingStateSet.HistoryNode and 
ConstrainedTracingSet.ConstrainedHistoryNode as independent classes.  They are 
not really a part of only the tracing state sets, and it makes testing 
difficult as they are.
