Duplicate community memberships

Describe the bug It was discovered a small number of users had duplicate community membership records which broke the UI. Limit creating a record to only if access isn't granted already.

To Reproduce Steps to reproduce the behavior:

1. Go to '...'
2. Click on '....'
3. Scroll down to '....'
4. See error

This wasn't closed by the above PR, but a constraint was added to make sure you don't duplicate a community membership
