@LoneDev6 I just solved the issue using a item based on a DIAMOND instead of FEATHER. Maybe it's the source of the issue.
Maybe, probably the game is somehow removing the item in this case, if no vanilla repair recipe is available (DIAMOND can be used in vanilla anvil repair recipes, FEATHER not).

What happened?
Consum all ingredients when repairing an item. If 4 ingredients was sufficient and you put more, all of ingredients will be consumed instead of 4 required.
