Global Search #30

As any user, I can search the entire system:

[x] Single search box
[x] Match on partial provider name
[x] Match on partial provider address
[x] Match on partial practice address from provider action
[x] Match on NPI
[x] On result click, go to provider or action detail page (based on where match was)

I think Amanda was working on this... if my memory is correct.

@bengm The way the search works now, it's searching from the server side. As of right now, @jobrieniii hasn't given the capacity to search by address

@mandy6720 - please let me know if this is still an open issue. (BTW, I don't see this as Global Search, I see this as searching for a provider while creating an action....) cc: @bengm

Can just be a provider index: add item to nav bar "Providers". Search bar at top, index result table below.

The first cut looks good. I would suggest: move the search box to be above the results table with full width (rather than corner), especially since this is the only thing on the page at first. It would be swell if it could be auto-search (on search term change vs. on button click).
