Update and add contributing templates #19

Changes:

  1. Update ISSUE_TEMPLATE.md
  2. Update PULL_REQUEST_TEMPLATE.md
  3. Add CONTRIBUTING.md

    Change details:

  4. To provide better way to separate sub-headers from text and also some additional information for commiters in comment.
  5. To provide better way to separate sub-headers and add references that automatically closes fixed issues.
  6. Add Contributing guidelines for issues and PRs, so they have some structure.
