changed fields of Add Document page according to the Trello Card. #38

Should we add some condition/filter for applying these changes to only http://contemporary.lacunastories.com/ ?

Actually, we want to preserve this as a separate branch entirely. We won't be merging this into master. It looks good, though. If you could update the changes so that there's a "Contemporary Content Types" feature separate from the default content types, that'd be a big help.

Since we have a new branch for this already, it seems reasonable to close this Pull Request.
