Remove `data_sources.record_download_option_provided` column #498

Context

The following columns in the database are strong candidates for removal:

record_download_option_provided: Never filled with data and does not exist in the data dictionaries
data_source_request: We now have alternative means of linking data sources with requests, and so this column is not needed

Remove from database and API schemas

What docs should be updated? Link to related docs changes in the PR.
