The Google Form should be:

Easy to read.
Clear and easy to fill.
Detailed enough to learn from it.

We should understand:

Who is the person that filled this form.
How much he likes sports and which types of sports he takes part in?
Would he use a system that connects people into a sports event?
What will make him use our system?
