How-to for accessing sql server with FDW #223

Related to https://github.com/PostgREST/postgrest/issues/1214.

Using sql server with docker and creating an FDW is actually simple to show.

I've used tds_fdw before and it worked pretty good. We could add this as a how-to and allow users to extrapolate for other dbs. Or of course if contributions come for other dbs later that would be highly welcomed.

This seems to be out of scope for PostgREST.
