Advise authors to point to the official version #1515

Documentation should advise authors to include (in the preface or colophon or wherever) a statement of where the official (and free) HTML version can be found. (For example, at the URL described in issue #1514 .)

This helps deal with unauthorized (or friendly but distracting) versions.

Currently the colophon allows a website entry, but does not allow a p that could be used to add an explanation about the official version. Simple inclusion of a website might not be sufficient to explain why the website is being provided.

A possible solution could be to allow a description for a website, and perhaps allow more than one website. (E.g. for APEX Calculus we might want to point to both www.apexcalculus.com, and explain that this is the main project website, and also to opentext.uleth.ca, and explain that this is where HTML versions of the book can be found.)
