Switch for outcomes #419

Processing switch to hide (in a closed knowl?) outcomes. Useful if you want them to appear in a "teacher's edition" but not in the student's edition. Not that you want to hide them from students, but if you feel they get in the way of the flow of reading.

Since the marginal extra work is small, some people woudl appreciate a "not there at all" option, in addition to visible and born-in-a-knowl.
