CSS for solution knowl clickables

Dear David,

Hints, answers, solutions (to exercise, example, task, etc) are always knowls and, iirc, we will have them lined up in a div.kohere once we finish that work. They have always (possibly) had simple numbers, now as of #764 they possibly have titles.

I have marked them up as span.type, span.codenumber, span.title, where latter two are absent if there is nothing to use. They are not wrapped in anything like h6 or header since they are just the "clickable" of a knowl. No spaces, though maybe we want to start allowing that so that they will render OK if no CSS?

Please advise if the markup needs changing or additions. It'll be trivial to adjust.

Live example is Exercise 16.3. You can compare with PDF if necessary (we shouldn't need parentheses in HTML, I'd think).
