CSS for notation table #827

Continuing with #732 to rid HTML of inline style. Nailing six here. Example:

Three th.notation-list-head were formerly text-align:left;

Three td.notation-list-entry were formerly text-align:left; vertical-align:top;

If you think the three columns need different treatment, let me know and we can have six classes instead of two. I'll make the change live once I hear that basic CSS is in place.

Notation lists seems to be okay.
