Increase PHPStan coverage #1167

Let's try to bump up PHPStan coverage by a level or two, for improved maintainability going forward.

Increasing to level 6 will be hard because of the required specifications on return and parameter types, especially complex arrays. I'm going to close this for the purposes of the 5.6.0 release and we can revisit in the future.
