Fixed minor bugs on the submission/upload page

Summary Change Description

Note that I also updated the submission google form (already live).

• Changed the submission doc link (in the example env file) - Note, this will need to be changed in the .env file after deployment.
• Changed the settings to get the submission details from the env file
• Fixed the upload page background (light gray to white)
• Fixed the expand/collapse icon updates

@lparsons, would you mind taking another look at just the new commit? I elected for a javascript alert for when the env vars are not set. Not all browsers support that from href=javascript:..., so I used a little template code to convert it to onclick. I tested it and it works, but if you have a better suggestion, let me know.
