Added Reset functionality in feedback form

Hey @Priyaaa1, issue closes #518

Issue Description The feedback form on the website did not reset after submission, leaving the input fields populated with the user's data.

Fix Implemented

• Added functionality to reset the form fields after successful submission.
• Ensured a clear indication of form submission and prepared the form for new entries.
