Git and Issue Linkup Management Failures

I have seen several Pull requests to have open state while the corresponding issues are marked as resolved fixed for the particular merge signal. could any one let me know the process over this or this is a fault?

There might be the two points behind this thing

  1. Issue is fixed on a given release but was not merged due lack of some priorities.
  2. Issue was fixed for another issues fix and was merged with that issue.

If first is the case then its a very critical case, if not then second option is the fix. for the second case we would be required to mark the issue as fixed.
