2. after login in to the page select the refer enter the data click the send button, in mail click the URL patient summary is shown but that page full data is not shown in patient summary at refer. like Any Other Procedures, Outcome this field data is not showing in refer patient summary.

3. AKI Registry --> patient summary --> investigations --> serology --> remove the dates in front of serology in patient summary .
