Clean up Issues vs Discussions; fix capilization #270

This PR incorporates suggestions made by @erogluorhan in #267.

It also applies standard capitalization to the terms "Issues", "Discussions", and "Pull Requests" wherever these terms are referring specifically to features of the GitHub interface, as raised by @jukent in #214.
