Bugfix/ph gate #151

Fixes a previously missed bug that Ph gates need to be applied to single qubits.

This should also fix errors which appeared in forks such as https://github.com/quantumlib/OpenFermion-ProjectQ/issues/11 https://github.com/quantumlib/OpenFermion-ProjectQ/pull/14
