This PR fixes 2 problems encountered with Diegesis.Bible:

• SOFRIA rendering failed when a w wraps other markup. I don't think this is legal USFM but eBible seems to do it a lot. I've 'fixed' this by generating an additional w on the fly.
• SofriaRenderFromProskomma wasn't generating chapter and verse callBacks for each new paragraph.

I added some tests which pass.

Also need to bump the version to (maybe) 0.6.4.
