The game crashes when testing video speed, which is required in the initial setup. I can't find any real solution for this, but you can get past it by editing line 17 in CONFIG.INI to Pass=1. Then you can edit the other settings in-game, but testing video speed will always crash, so that option has to be manually edited in CONFIG.INI (I changed it from 5000 to 139945, which is used by GOG). This file is included in the DOSZ so it's playable on its own
Prior to the crash in initial setup, for digital I used Sound Blaster 16 at port 220, interrupt 7, DMA 5; and for midi, Sound Blaster 16 at the default port 388
DOSC has the required CONFIG.INI file and auto start
GOG variant can't be added as it uses a no-CD setup that relies on mounting the C drive as the D drive
