Make updates to ticket form

Specifically:

• add phone number field on ticket sales form
• add meal ticket at £45 per person
• address the concessions ticket for students, unemployed etc...

These have all been done. Still waiting for clarity on the concession modus operandi and I'll leave this open until it's all deployed.
