Usage of lower-case / proper-case named elements #1

Some element names start with Capital letters while others don't.
Document the naming policy for elements in Akoma Ntoso, and the
significance of starting element names with capitals.

lowercase elements are concrete elements.
Uppercase elements are abstract or generic elements.

You are right, this naming convention should be mentioned in the documentation, 
as well as the distinction 
between plural and singular elements. 

Also, we should check whether I consistently followed this convention.
