This HAS been documented.
The table the element synopsis is correct. Section 12.4 now reports title as
another hierarchical element, and
heading as a title element. The second and third bullet point in Section 16.1
(differences from the previous
version) also explain what has happened and why.

Here we may have had to use "request for improvement" instead of "defect" as
label.
My apologies for that.
I may be wrong, but I noticed that:
1) in the element title it is stated "Example: see element body" but in the
element
body I did not see title. Did I look in the wrong place?
2) in the element heading there is no "description". Is that intentional or did
we
deleted somehow?
Please consider to change the label from "defect" to "request of improvement"
and the
status from "invalid" to ... I do not know .. work in progress .

1) We do not have examples for all elements. It would be impossible. The
element title is just another name for
an element of the main hierarchy. The example in the element body has elements
drawn from the hierarchy. Not
all elements. Just some. Their use is identical.
2) You are right. A description has now been given. It will be available at the
next iteration of the release notes
