Add local mode to Qiskit Code Assistant Guide #2275

this PR adds instruction for running the Qiskit Code Assistant locally.

It’s a good start! Some comments. Most of them are to distinguish the model name vs the service name and to reinforce the name for the model instead of using a generic way to refer to it
