Set up Terra infrastructure to deploy docs #1779

Part of https://github.com/Qiskit/qiskit-metapackage/issues/1722.
The key is that we'll deploy to some private site on qiskit.org.
It's important that this sets nbsphinx_execute to always so that we execute tutorials.
