Add the `qiskit-ecosystem` theme #506

This makes the _qiskit-ecosystem now public.

It also documents how to use it - closes https://github.com/Qiskit/qiskit_sphinx_theme/issues/486. Note that the default theme should be qiskit-ecosystem; only Terra should use qiskit.

Also fixes some broken links in the README - closes https://github.com/Qiskit/qiskit_sphinx_theme/issues/488.
