Remove `qiskit` theme #584

Closes https://github.com/Qiskit/qiskit_sphinx_theme/issues/577.

This allows us to greatly simplify the repository. Now we only have a single theme, whereas before we had 3 themes so had to generalize a bunch of logic like our visual regression testing.

Closed in favor of https://github.com/Qiskit/qiskit_sphinx_theme/pull/585 so that I can confirm live docs previews still work.
