The link you gave already has table of contents. Maybe you got the wrong link?
Sorry for the poor description. I meant a table of contents with links (Hyperlink package I believe). So that one can also have the table of contents on a separate docked window on your favorite PDF reader.
I guess the issue is that we first create a DVI... This is handy to include EPS images and create the html output (which is outdated) but seems to break the table of contents.
I see, I will try this when epstk website is back. Since I cannot compile technical.pdf without epstk and it's official website is down with unknown reason lol -- OK I got epstk from debian package, I will start fixing this issue.
