the toast popup appears even if Firefox is not in the foreground
The visibility of those notifications is controlled between Firefox and the operating system. All the add-on does is "show a notification that says this" and Firefox takes care of the rest. I'm not sure about how it should behave in Linux, perhaps that's expected. I know that in Windows that doesn't seem to happen. I suggest you open a bug in bugzilla to ask Firefox's devs about it (ping me in it), as there's really nothing I can do about that.

Do you mean the checkbox in the add-on's About page, "Don't notify me on the next update"? That should never be reset automatically at any point. Are you running some kind of cleaning software, or somehow erasing/cleaning your profile folder?
I just experienced the same while watching a video on Amazon Prime (probably html5). I guess the behaviour is normal though.
A minor but jarring issue with the update notification popup. I'm on Ubuntu 16.10 and whenever the plugin updates the toast popup appears even if Firefox is not in the foreground. This will even happen over the top of fullscreen applications. Please consider changing so it respects the application in the foreground. I'd disable the popup but the preference seems to get reset periodically. Thanks for all your hard work on keeping tab groups alive!
