Add support for NHS/web data input

• Add support for NHS/web data input
• This allows the user to open an external URL, get information from that URL, then input this back as text input in the app
• Add notes input for larger text input (also called notes in REDCap)

For the Notes I wonder if it is a good idea to have a structured field (e.g. multiple select) for some standard answers then the text field if they select "Other"? This will be easier to analyse than pure text (thought the text is also valuable). We should discuss what standard answers might be for this but some starting ones might be:

• Concerns about privacy
• Concerns about stigma and discrimination
• Legal and regulatory uncertainties
• Other
• Prefer not to say
