Additional loans fields #68

add number of months for extension and reason for extension fields - These fields are currently not present in the loans form and the user would like them added.

This was part of #38 which included a large number of seperate issues. Splitting it out to make it easier to manage

Number of months of an extension could be included - there is currently a spare integer field.

There is a remarks field and the reason for the extension could be included here.

Robyn to add extension length field to forms. Need to include use of the remarks field to record reason in the documentation
