Update of gt disposition table #116

Created new design of gt disposition table and cleaned up code for conmeds.

This update and #115 result in quite different tables. I don't know which design would be preferred.

So sorry, @dmurdoch. I was working off of the reference in #110 and missed the discussion yesterday. If @gmbecker and you found a consensus on the tables version, I can update. Just let me know.

I don't think we discussed this change yesterday. I don't really have a preference for one format or the other, but I'd like to settle the issue quickly. We agreed in January or February to have the book finished this month, and we did confirm that decision yesterday.
