Add precision field (and maybe also certitude) to patientValue #220

Close this ticket when:

  1. how to define precision has been decided (decimal points? Interval?)
  2. add doc for precision to pds-cdm-specs yml and to each of the source codes that create/handle a patientvariable value
  3. add an example for precision to pds-cdm-specs yml, and be sure that yml is propagated to all hte plugins that include a PatientVariable type
