Ci/simplify pull only deploy #74

Separate deployment templates for extracting from DB (only) or "batteries included" (Extractor, DB, and Data Loader).

Managing these in a single template file has been finicky, and we do test extractions (usually from PRs) on existing databases frequently enough that having that process be reliable is useful.

The updated commands in READme point to the new templates and param files, and deploy cleanly.
