Add item upgrading plugin #41

Allow players to upgrade items to create new items with better quality. Requires #33

Add item upgrading ladders, formed of item qualities. These give the different qualities an order to be upgraded in. Add a method of upgrading: via an anvil, for example. This should upgrade the quality on the item to the next one, if available in the ladder.
