Issue templates & emails for course setup #35

I thought I'd already done this but it appears not.

Adding Issue Templates and emails (shamelessly copied from Git, GitHub & GitKraken repo) to aid setting up the course.

At some point I'll get a template for using the eventbrite package @davidwilby has written.
