Respondents versus ID #49

The first column in the _C file is described as Respondent. We should call it ID to be consistent with the input and the fact that RSGHB could be used for non-survey data.

If you go with "ID", just know that people trying to open/work with the data in Excel will get the annoying "file format is not valid" error from our good friends at Microsoft...

Lower case "id" could be a good alternative.
