Add surgical margin and it being positive/negative #64

E.g. Circumferential Resection Margin (CRM) -> pos/neg Intestional Margin -> pos/neg/close

Added from NCIT -CRM -Negative, positive, narrow. -Macroscopic positive, microscopic positive. Intestinal margin not modelled as a separate concept.
