Port of dicom-html extension of OHIFv2 to OHIF v3.
OHIF v3 has cornerstone-dicom-sr extension to deal with SRs. But this focuses on Measurement SRs, and do not support text-only reports.
On the other hand, dicom-html focuses on text-based reports.
NOTE: for now, two extensions are not compatible, and should not be used within the same mode.
