Can you provide more details of exactly what you want?

Note that if you add title="something" to your heading tags then the TOC editor 
will show the contents of the title instead of the text between the heading 
tags.

Since you can add title="something" to your H tags there is no need for a 
rename function just for this, e.g. <H2 title="something"><img src="..." /></H2>
