Add option to keep the manually added TOC items when "Generate TOC from headings"

Would be great to have an option to keep the manually added TOC items when 
"Generate TOC from headings".  Say if i missed a heading or decided to add more 
headings, and use the "Generate TOC from headings" to regenerate the TOC, I 
would lose all manually added TOC items and have to reinsert them and then do 
the renumbering all over again.

Either that or add an option to include genueine <a>....</a> links into the TOC.
This would allow us to add such links on pages where no visible heading is 
available and still get something in the TOC each time it is generated!
