Generate TOC from <a> links and headings #1262

It would be nice to have an option to add <a>....</a> links to the TOC when 
generating the TOC from headings.
This would allow one to add such links on pages where no visible heading is 
available and still get something useful in the TOC each time it is generated.

Just add a heading tag around the anchor.
