Table of Contents didn't get cleared #1279

What steps will reproduce the problem?
1. Create a new epub with several pages, each page with a header, I chosed h3.
2. Click "Generate TOC from headings" to generate the TOC.
3. Save.
4. Open a different existing epub that you know does not have TOC, note that 
the TOC from the previous epub is still visible. Clicking on any of them bring 
up the error alert "The xxx page does not exist"

What is the expected output? What do you see instead?
TOC should be empty.

What version of the product are you using? On what operating system?
0.5.0

Unable to reproduce in 0.5.3 on Ubuntu 11.10

1. Started Sigil, added 1 line of text, saved as plainfile.epub
2. Restarted Sigil, created 4 pages each with 1 line marked as h1, clicked 
generate toc, clicked save as
3. Opened plainfile.epub -> TOC cleared as expected.

If you have sample epubs where this re-occurs please attach and we'll re-open.
