We are currently trying to figure out how to better get this to work. PArt of the issue is that Mistlands changed colliders for a lot of the ground elements, and now stuff spawning in doesn't know about it. Please note we are aware of this issue.
Dear developer, on one wonderful server your mod is actively used. And we are terribly tired of the fact that monsters and their companions and chests are either in textures or somewhere in a parallel world. If possible, please fix it, as your mod is very important to us, we will be very grateful to you.
